What the ANCC exam testsโถ
Advanced Practice Skills is the highest-weighted domain on the ANCC PMHNP-BC exam at 27% of scored questions. This domain tests your ability to conduct a comprehensive psychiatric evaluation, perform a mental status examination, select and interpret appropriate screening instruments, and formulate clinical assessments based on gathered data.
Expect questions on clinical interviewing techniques, risk assessment for suicidality and violence, collateral information gathering, and case conceptualization. The exam tests whether you can synthesize subjective and objective data into a coherent clinical picture and identify the most appropriate next step in the diagnostic workup.
You will also be tested on health history components specific to psychiatric presentations, the role of laboratory studies and neuroimaging in psychiatric assessment, and how to distinguish psychiatric disorders from medical conditions. Cultural competence in the assessment process is a recurring theme.
Common mistakes to avoidโถ
- โRelying on a single screening tool when the clinical presentation warrants a broader assessment battery. The PHQ-9 screens for depression severity but does not rule out bipolar disorder; the MDQ or a careful mood history is needed as well.
- โJumping to a diagnosis before completing the mental status examination. The exam tests systematic assessment: appearance, behavior, speech, mood, affect, thought process, thought content, cognition, insight, and judgment.
- โOverlooking medical mimics. Hypothyroidism, substance intoxication, and delirium can present with psychiatric symptoms. The exam expects you to order appropriate labs before attributing symptoms to a primary psychiatric disorder.
- โUndervaluing collateral information. Family and caregiver reports are critical for accurate assessment, especially in pediatric, geriatric, and psychotic presentations.
- โFailing to perform structured risk assessments. The Columbia Suicide Severity Rating Scale (C-SSRS) and similar tools are expected knowledge for determining risk level and appropriate level of care.
