Clozapine monitoring is multifaceted: ANC per the prescribing information schedule, metabolic monitoring (clozapine has the highest metabolic risk of all antipsychotics), cardiac monitoring (myocarditis risk is highest in the first month but cardiomyopathy can develop later), liver function, constipation assessment (clozapine-induced constipation can progress to ileus, which is potentially fatal), and general side effect screening.

No monitoring is needed at this visit since the patient is asymptomatic, clinically stable, and reports feeling well with no psychiatric or physical complaints.

Clozapine monitoring is required regardless of symptoms. The most dangerous complications (agranulocytosis, myocarditis, metabolic syndrome) can develop without obvious symptoms.

Only metabolic labs are needed at this point, since the labeled ANC monitoring requirement ends after the first year of continuous clozapine treatment.

ANC monitoring continues for the duration of clozapine treatment. The frequency decreases over time but never stops while the patient is on clozapine.
