A 4-year-old boy is brought in by his mother for frequent tantrums, defiance, and aggression toward peers at preschool. He meets criteria for oppositional defiant disorder. The PMHNP refers the family for a therapy in which the mother wears a wireless earpiece while playing with her child, and a therapist coaches her in real time from behind a one-way mirror. In the first phase, the mother is instructed to follow the child's lead in play, describe his actions, reflect his verbalizations, and offer specific labeled praise while avoiding questions, commands, and criticism.
